Attention all league directors

The Youth Activities Fund Committee is currently accepting funding requests from local organized leagues for FY 2012/2013. In order to participate in the Youth Activities Fund Voucher Program, each league must submit the following:

1) By-Laws of the organization

2) List of Board Members with contact information

3) Tax ID number

4) Most recent financial report

5) Number of children assisted with previous funding

6) Certification stating that no child will be turned away for inability to pay for registration.

The funding is not issued to the participating leagues directly; however, low-income families may apply for a “voucher” at the City of Douglas Neighborhoods Division.

Vouchers shall be limited to $50.00 per player, per sport and will not be issued for more than two sports per child/per year.
